Our interdisciplinary team of experts is made up of healthcare professionals who work together to provide comprehensive, holistic care for the patient and their family. This may include a doctor, nurse, social worker, chaplain, CNA, and other specialists as needed.
Yes, you can change your mind about hospice care at any time. Our team will work with you to understand your needs and preferences and make changes to the care plan as needed.
Medicare covers 100% of hospice care costs as long as the patient is eligible for Medicare Part A.
Do you have an insurance that is not Medicare Part A? Contact us at (630) 366-0168 to see if we are in network with your insurance company.
Hospice services typically cover medications and medical supplies related to the primary hospice diagnosis and those needed for comfort, and medical equipment (oxygen concentrator, hospital bed, overbed table, wheelchair, etc).
Vitamins and supplements are typically not covered by hospice services. Some medications not related to the hospice primary diagnosis may not be covered. Hospice does not cover any aggressive treatments.

* Last physician visit note/hospital discharge summary or medical records
* Facesheet or address and insurance information
Accreditation: Pioneer Hospice Care is seeking accreditation from CHAP (Community Health Accreditation Partner).
Certification: Pioneer Hospice Care is seeking certification from Medicare.
Pioneer Hospice Care is also seeking "Age Friendly Care at Home" Certification from CHAP (Community Health Accreditation Partner).
Licensing: Pioneer Hospice Care is licensed by the State of Illinois and Registered with Kendall County.
Hospice care is available to individuals who have been diagnosed with a terminal illness, or chronic disease that is life-limiting, and have a life expectancy of six months or less if the disease runs its usual course. Patients must also have a physician's certification of their terminal illness and must agree to discontinue curative, aggressive treatments.
Pioneer Hospice Care limits its patients to adults only, and does not provide services for pediatric ages.
Federal guidelines require all Hospices care agencies to provide a wide range of services, including pain management, symptom control, emotional support, spiritual care, and counseling. Pioneer Hospice Care provides additional specialized services to provide optimal comfort, support and joy to patients and their families.
