A Living Will gives direction on what treatments the patient would or would not like if he/she is not able to speak. This document must be signed by the patient.
A Power of Attorney gives permission for a person to make decisions for a patient who cannot make decisions for him/herself. This document must be signed by the patient in front of a Notary Public.
A POLST form (aka DNR) tells medical personnel what heroic measures (if any) should or should not be taken if the patients is not breathing and has no pulse. This document requires the signature be witnessed and the physician sign as well.
